Returning Students:
• Valid Parent ID
o Driver’s License
o State ID
o Military ID
o Passport
• Proof of Residency – ONE of the following:
o Current Utility Bill (electric, gas, trash or water - NO cable bill / NO phone bill)
o Current Mortgage Statement
o Current Rental Lease
o Current Property Tax Statement showing Homestead Exemption
o Residency Verification Home Visit by RISD employee (forms available on site). Parent and student are bone fide residents of the District, i.e., that they eat, sleep, and maintain clothing and other personal effects at an address within the District’s boundaries.
o Residency Verification Notarized Form - parent/child living in someone else’s home (forms and notary will be available on site at no charge – requires parent’s signature and home owner’s proof of residency and home owner’s signature). Parent and student are bone fide residents of the District, i.e., that they eat, sleep, and maintain clothing and other personal effects at an address within the District’s boundaries.
o McKinney-Vento Form (form available on site)
• Updated/Current Immunization Record (Nurse will also check ImmTrac)
• Foster Care Documentation, if applicable (Form 2085 or DFPS Letter)
• Court document(s) affecting parent custody/access that has not previously been submitted, if applicable
• Pre-K Students Only
o Proof of Income for BOTH parents (latest check stubs) and
o child support (if applicable)
or
o any CURRENT Snap or TANIF with EDG#
or
o Foster Care Documentation (Form 2085 or DFPS Letter)
or
o Military Connected – Documentation for child of an active duty, injured or killed member of the armed forces.
New Students:
• Documents listed above
• Certified Birth Certificate
• Social Security Card (if available)
• End-of-year report card from previous school for grades KG-12th (to ensure accurate grade level placement) or current transcript for grades 9th – 12th
All required documents must be submitted and immunizations must be up-to-date before your child is placed in a classroom or assigned a schedule. It is the parent’s responsibility to turn in all required documents.
